Calculating Amlodipine IV Dose from Oral Dose

There’s no direct, universally accepted conversion factor for amlodipine from oral to IV administration. Dosage must be individualized and carefully titrated based on patient response and clinical judgment.

Begin with a lower IV dose than the equivalent oral dose. A common starting point is approximately 1/10th to 1/5th of the patient’s usual oral daily dose given as an initial IV bolus. For instance, if a patient takes 10mg orally daily, a reasonable starting IV bolus might be 1-2mg.

Closely monitor blood pressure following administration. Frequent blood pressure checks are crucial for safe titration.

Adjust the subsequent doses based on the patient’s response. If the desired blood pressure reduction isn’t achieved, cautiously increase the dose by increments under close supervision. These increments should be considerably smaller than the initial dose adjustment.

Remember: IV amlodipine has a shorter half-life compared to the oral formulation. This necessitates more frequent dosing, possibly requiring continuous infusions in some cases, depending on clinical need and patient response.
